I knowingly waive genetic testing for purposes of this acknowledgment, unless otherwise required by law or ordered by a competent authority.
• To the best of my knowledge and belief, no other individual qualifies as a presumed, acknowledged, or adjudicated father of the child under the parentage laws of [Insert Governing Jurisdiction].

4. Legal Consequences

Cuarta.
